I perform about 700 breast augmentation procedures annually. Let me tell you what I have learned from my experiences. Two of the most important decisions are the size and shape of the implant. I believe that the surgeon should assist in the shape decision but limit input into size when he/she sees that it is not proportionate to the patient's body. I have found that the patient's desires are the most important criteria. When you discuss implant size, forget about cups or CCs; these can be arbitrary and confusing. When I do a surgery I try to create a look based upon the patient's input. This is best determined by looking at previous patient results or sample pictures from magazines. The women can be in clothing or a bathing suit. If the look can be conveyed to me I am very confident in picking the right size and shape to make my patient happy. I will take some measurements during the physical examination in the initial consultation. These measurements are only guidelines and do not usually determine the implant size though they can influence the shape depending upon the look each patient desires. For instance, if a patient is going for a larger "look" but has a relatively narrow chest I might suggest using a high profile implant. This will give a well defined cleavage without it being too close or extending laterally in the chest; the inverse relation is true in that a low profile implant may be chosen to allow for a good cleavage approximation. I know that there are surgeons who dictate the implant sizes for their patients; I believe that this is a mistake because only the patient truly knows what is best for herself and her lifestyle. I think the surgeon should only influence the decision in situations when a certain implant size is not possible due to physical limitations.
